We understands changing provider can be an anxious time for all involved. We’ve worked closely with the people we support, families and staff to develop a robust and supportive transition process.
Transition success factors
Our experience coupled with feedback from people we support, their families and our staff shows the key success factors for a smooth transition are consistency, reliability and open communication.
Our mobilisation process
We have a dedicated mobilisation team who facilitate smooth transitions that are outcome-focused and person-centred. They create transition plans that cause minimal disruption for all through:
- Working in partnership to establish working protocols and transfer existing support plans.
- Enabling open and timely communication through meet and greet events.
- Developing positive relationships with key stakeholders and healthcare professionals.
- Follow person-centred planning processes.
- Using established HR processes.
An example of a recent transition
During 2016 we worked in partnership with Metropolitan Housing Trust to successfully transfer eight services (seven residential and one supported living) in Cambridgeshire, supporting 49 individuals and employing 70+ staff. Ann Gibbons worked with us during the transfer.
I am happy to recommend Voyage Care based on my experience in transferring services to them during 2016. We worked closely with Voyage Care to transfer eight registered and supported living services from Metropolitan. Throughout the process we found the Voyage Care team took a proactive and pragmatic approach and were professional at all times. The transfer was completed in the agreed timescales and, more importantly, allowed continuity for both the staff team in Cambridgeshire and the care of 49 individuals living in the services.
