12 Channel Lea is a residential care home in Walmer, Kent, providing support for up to three adults with autism, learning disabilities, mental health needs and low levels of challenging behaviours.
Supporting you
Our dedicated team work closely with the people we support to create person-centred care plans and positive behaviour support plans, enabling tailored and consistent support.
Our focus is on fostering independence, empowering and supporting individuals to develop their daily living skills, giving them the confidence to step-down into supported or independent living arrangements should they wish to do so.
The people we support are encouraged to create their own weekly programme of activities aligned with their interests and goals. Our home has built strong links with a local adult education group, as well as numerous clubs offering a wide variety of activities and opportunities to socialise and gain new skills.
Facilities
Our four-bedroom home has a welcoming and friendly environment, with a communal lounge and a spacious kitchen/diner to enjoy cooking and eating meals together. We have a communal bathroom upstairs as well as a shared toilet downstairs for personal care.
We also have a lovely garden where the people we support can enjoy spending time with nature!
Our team
At 12 Channel Lea, we ensure the delivery of high quality, person-centred care. Our expertly trained team work with the people we support to ensure their needs and interest are at the centre of their care plan, encouraging them to live as independently as possible. This includes supporting individuals to go bowling, on cinema trips, shopping, gardening and family visits!
