We are experienced in enabling people to move from residential settings to supported living as their confidence and independence grows. Whether you have a personal budget or are funded by your local health or social care team, we can support you:
- in your home on a one-to-one basis,
- to access activities or day services in the community,
- with short breaks, respite or emergency cover,
- with overnight support.

- Shared living - we can help connect you to other people who might want to share their home

Our approach to transition
At Voyage, we believe that every person has the right to live the life they want – we help support people to live where, how and with whom they choose.
Your support team will not only help you to do the things in life you enjoy, but can be there for you and your family whenever needed. We understand that transition plans should be personalised and tailored to your needs so that you and your family are comfortable at each stage.
We will work with you and the people who are close to you to develop a tailored transition plan. We are flexible and will review and change plans as needed to reduce any concerns you may have.
The length of time transition takes will depend your circumstances and wishes. We can support you to get changes in place quickly when necessary, or you may prefer to have a plan in place well in advance.
We can work closely with your current service provider where appropriate to make sure existing support plans and positive interventions are mirrored in the new service. Our staff can spend time working alongside the people who currently support you to see how you prefer to be supported and give you time to start to get to know each other.
We will give you the opportunity to visit your new service regularly with the option of overnight stays before you move in. We are aware that someone new moving into a service also affects the people who already live there and we make sure we support them with any questions or concerns. Our pre-placement assessment includes compatibility checks to ensure the right fit for you and anyone you choose to live with.
